  • Case management
  • Orientation to self-help programs
  • Develop individualized service plans
  • Support with housing applications/accessing resources
  • Assisting residents on or seeking MAT
  • Opioid overdose prevention education
  • Referrals to appropriate community services
  • Individualized aftercare planning (includes staff assistance with placement in permanent, sustainable housing)
  • Referrals to groups on substance use & recovery skills, relapse prevention/management, life skills, health/wellness and more
  • Access to medical & mental health services
  • Family involvement encouraged, including assistance with navigating DCF cases
  • Assistance with addressing legal issues
